PHARMACIST
PERMANENT PART-TIME (0.5 FULL-TIME EQUIVALENT)

The Pharmacist will focus on providing education and counseling services rather than dispensing medications. Key responsibilities include conducting compliance assessments, performing medication reviews, and delivering patient education. The Pharmacist will also support and educate healthcare professionals, manage drug costs, and oversee the safe destruction of medications. This role is essential in optimizing patient care through informed medication management and support for the healthcare team.

Qualifications: • Bachelor’s degree in Pharmacy with one year experience directly related to the duties and responsibilities specified. Must be accredited by the Ontario College of Pharmacists.
Knowledge and experience working in collaboration with community health organizations to promote health and well-being programs is an asset. As a condition of employment, a vulnerable sector police clearance is required and you are required to submit proof of COVID-19 vaccination.

POSITION DESCRIPTION
Job Title: Pharmacist

Reports to: The Nurse Practitioner Clinical Lead

Status: Permanent Part Time (0.5 Full Time Equivalent FTE)

Summary:
The Pharmacist will focus on providing education and counseling services rather than dispensing medications. Key responsibilities include conducting compliance assessments, performing medication reviews, and delivering patient education. The Pharmacist will also support and educate healthcare professionals, manage drug costs, and oversee the safe destruction of medications. This role is essential in optimizing patient care through informed medication management and support for the healthcare team.

Accountability:
The pharmacist will be accountable to the Nurse Practitioner Clinical Lead and to their professional body for the standard and quality of service provided in their clinical responsibilities.

Duties and Responsibilities:
• Counsels clients and/or medical practitioners on drug indications/contraindications, doseage, drug interactions, side effects; provides other drug and pharmaceutical information as appropriate.
• Takes clients’ histories as required; makes initial clinical assessments and refers clients to appropriate clinicians.
• Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
• Provides guidance and training to pharmacy interns during course of work and monitors performance.
• Participates in health promotion programming.
• Facilitates safe destruction of drugs, and sharps.

• Due to the nature of the position, flexible hours are required including evenings until 8:00 p.m. You may also be required to work out of either of our office in Chatham or Tilbury.
• Practice within the scope of a Pharmacist.
• 3 to 5 years of experience in a primary care setting is an asset
