Current patients *PLEASE NOTE* – If your former physician at the Tilbury District Family Health Team is no longer with our office, please visit the office to complete the necessary paperwork to enroll / roster with your new physician.

If you have recently signed a “roster” form during a visit since June 2007, this is sufficient.
Patients will have access to primary health care treatment advice, 24 hours a day, seven days a week, through a combination of regular and extended hours, and a nurse-staffed Telephone Health Advisory Service (THAS).
THAS is only available to patients who are rostered to a Family Health Team, aiming to improve access to primary health care services when the office is closed.
