Complaints are taken seriously at the Tilbury District Family Health Team. To ensure that they are received in a timely and orderly fashion, please follow the following steps to file a formal complaint.
Formal complaints procedure:
1. Complete the “Complaint Report” below and click “Submit”.
2. Our Clinic Manager will then meet with the management team to review your complaint.
3. If needed, your complaint will be reviewed with other healthcare team members. In some cases, we may ask consult with you as we review the complaint.
4. If you would like a response from our management team, you will be contacted within four weeks of submitting your complaint. If this timeline cannot be met, a member of our team will let you know when you can expect a response.
