Please visit the Tilbury District Family Health Team to enroll / roster with our Family Health Team. This Ministry process is required in addition to the application and medical history process recently completed for new patients.
Please note, enrollment forms must be completed for new patients and patients that switch to a new physician. Please contact Sharon Moore should you have any questions.
If you have recently signed a “roster” form during a visit since June 2007, this is sufficient.
For new patients, the enrollment form is separate from the application forms completed.
Benefits of Enrollment
Patients will have access to primary health care treatment advice, 24 hours a day, seven days a week, through a combination of regular and extended hours, and a nurse-staffed Telephone Health Advisory Service (THAS).
THAS is only available to patients who are rostered to a Family Health Team, aiming to improve access to primary health care services when the office is closed.
Patient Enrollment Requirements
By completing and signing the Patient Enrollment and Consent to Release Personal Health
Information Form, patients commit to the following:
To seek treatment from their family doctor/ nurse practitioner in the group first, unless they are traveling or find themselves in an emergency situation.
To allow the Ministry of Health and Long Term Care to provide their doctor with information about services they have received from family doctors outside the Family Health Team, and some preventative care services.
