Diabetes impacts many aspects of daily living. Our diabetes education team will assist you in learning the causes and effects of diabetes and important factors of the disease.

Education surrounds blood sugar monitoring and control, diabetes medications, healthy food choices, appropriate eye care, foot care, and prevention of complications.

Our diabetic education team is comprised of different members of expertise, including a Registered Nurse, Pharmacist, Registered Dietitian, and other Primary Care Providers.

Individual counseling appointments are available with our Certified Diabetic Educators.

This program is open to TDFHT patients by self-referral, or ask your doctor or nurse practitioner.
