Dr. Dhaliwal will be closing her practice at the Tilbury District Family Team due to personal reasons. A letter has been mailed to all of Dr. Dhaliwal’s patients.
Dr. Dhaliwal will continue to provide care both through in-person and virtual care until October 30th, 2021.
It is advisable that her patients find a new physician for future care as soon as possible.
Dr. Dhaliwal will be sending out enough prescription refills for 6 months. These refills need to be requested by your pharmacy.
Please check your prescriptions and ensure that your pharmacy faxes the refill requests.
Changing physicians can be stressful, but we encourage you to seek continuity of care by contacting Health Care Connect at 1-800-445-1822 for information on other family physicians in the area.

All medical records will be held at the Tilbury District Family Health Team. These can be transferred to your new primary care physician on request. Please send your request for the transfer of records to the Tilbury District Family Health Team.
Unfortunately, at this time, the other physicians at the Tilbury District Family Health Team have full practices and are unable to accept patients.
The Tilbury District Family Health Team will be supporting the efforts towards recruiting other physicians.
