The Tilbury District Family Health Team (TDFHT) is a team of health care professionals including doctors, nurses, social workers, pharmacists, dietitian etc. that work together to co-ordinate the highest possible quality of care for the patients.

TDFHT provides better access to care, closer to home; offers extended hours; delivers primary health care, chronic disease management and self-help tools to improve health.
