
The Tilbury District Family Health Team is excited to be a part of a system-wide initiative to improve access, equity and quality for our patients and communities. Our commitment to quality is etched in our mission “to improve health and well being through accessible primary health care services and programs”. Our Board of Directors through their leadership ensures quality continues to be a priority within our organization. The framework of our strategic plan is based on improvement which we define as “Excellence in All We Do”. Our strategic priorities are reflected in our operational and quality improvement plans.
Our team is beginning our sixth quality improvement cycle. In developing our 2018-2019 Quality Improvement Plan, our Quality Improvement Committee have incorporated the following key elements: timely access, stronger links to other health services, effective integration and greater equity. We have talented and competent staff, engaged community Board of Directors and strong community partnerships to support us with our quality initiatives.
Our Quality Improvement Plan reflects the distinctive needs of our patients and our community. The TDFHT has implemented strategies to engage our patients and incorporate their ideas and feedback into our plan. It is extremely valuable to have a patient’s perspective in the development and implementation of new change ideas. We also engage patient feedback through patient satisfaction surveys and focus groups
In response to patient feedback, we have begun to enhance our patients’ experiences and quality of care through on-going system improvements. The TDFHT continues to employ these actions to enrich quality of care and better our patients’ overall experiences with us.
