Quality Improvement

The Tilbury District Family Health Team is excited to be a part of a system-wide initiative to improve access, equity and quality for our patients and communities. Our commitment to quality is etched in our mission “to improve health and well-being through accessible primary health care services and programs.” Through the leadership of our Board of Directors, the organization ensures that there are processes in place to deliver and measure quality. The framework of our strategic plan is based on improvement, which we define as “Excellence in All We Do.” Our strategic priorities are reflected in our operational and quality improvement plans.

Our Quality Improvement Committee is a small group of interprofessional team members that champion our Quality Improvement strategies. The following dimensions are leveraged to advance quality initiatives: efficient, timely, patient-centered, safe, equitable, effective and focused on population health. We have a diverse and talented team, supported by a skills-based Board of Directors with strong community partnerships to support us with our quality initiatives.

Our Quality Improvement Plan reflects the distinctive needs of our patients and our community. The TDFHT has implemented strategies to engage our patients and incorporate their ideas and feedback into our plan. It is extremely valuable to have a patient’s perspective in the development and implementation of new change ideas. We also engage patient feedback through patient satisfaction surveys and focus groups.
